• 2019-2020 Student Transportation Information Form
- please fill out this form to notify the Chapel Hill Carrboro City Schools Transportation Department of a change to your child’s AM or PM ride status for the 2019-2020 school year.


• 2019-2020 Unsafe Stop
– complete this form if there is a perceived safety issue with your current bus stop. Clearly indicate the stops affected (morning, afternoon, or both). Include all details about the unsafe stop in the "reason of request" section, explaining conditions that support your concern. Do not use this form to request a stop relocation for personal convenience and/or to obtain a stop closer to the child’s residence.


• 2019-2020 Alternate Stop
– complete this form if your child needs transportation to a location other than
your home address such as a licensed daycare or after school program.

Requests for alternate bus stops, including licensed daycares and/or a relative's address, will be approved if the requested stop is within the student's school boundary. Only students eligible for transportation may request transportation changes. Please note that transportation services will not be granted to locations such as businesses (e.g., a parent's workplace), after-school activities or student's workplaces.
